This observation occurs because gallstones are formed over an extended period of time, resulting in a shrunken, fibrotic gall bladder which does not distend easily.
As the pressure increases and reaches a point where the abdomen can no longer be distended it starts to affect the cardiovascular and pulmonary systems.
